IMPORTANT: PLEASE READ EACH QUESTION CAREFULLY

FOR ACTIVE VETERINARIANS: I hereby attest that – 1.) I have completed the twenty (20) hours of continuing education required for renewal and have submitted those hours through the CE INPUT portal.* 2.) During the previous calendar year, I was not charged with or convicted of a felony, nor was I the subject of any disciplinary action by any government agency or another state’s licensing authority. 3.) I am currently not the subject of any criminal proceedings, or regulatory or licensure investigations. (Does not include Arkansas VMEB complaints, as that information is already on file.)

FOR ACTIVE VETERINARY TECHNICIANS/TECHNOLOGISTS: I hereby attest that – 1.) I have completed the twelve (12) hours of continuing education required for renewal and have submitted those hours through the CE INPUT portal.* 2.) My current supervising veterinarian is on file with the Board. 3.) During the previous calendar year, I was not charged with or convicted of a felony, nor was I the subject of any disciplinary action by any government agency or another state’s licensing authority. 4.) I am currently not the subject of any criminal proceedings, or regulatory or licensure investigations. (Does not include Arkansas VMEB complaints, as that information is already on file.)

FOR INACTIVE RENEWALS: I hereby attest that – 1.) During the previous calendar year, I was not charged with or convicted of a felony, nor was I the subject of any disciplinary action by any government agency or another state’s licensing authority. 2.) I am currently not the subject of any criminal proceedings, or regulatory or licensure investigations.

FOR CORPORATE PRACTICES: I hereby attest that the business entity is in good standing with the Arkansas Secretary of State.

*At its discretion, the Board may request an audit of the hours submitted; therefore, it is your responsibility to keep a record (proof of attendance) of your continuing education for three (3) years.
